Referrals for a Lambeth Local Authority child or young person
Permission must be sought from Lambeth Local Authority (LA) for funding before CENMAC can assess the child or young person.

Complete Referral Form 1 (please download a copy and save it) for advice on Assistive Technology (AT) for learning only.

Complete Referral Form 2 (please download a copy and save it) for advice on Assistive Technology (AT) for learning and Augmentative and Alternative Communication (AAC) for communication.
