Referrals for a Southwark Local Authority child or young person
Referrals for Southwark based children or young people must be made directly to the Southwark Local Authority (LA).
How to make a referral
To make a, please email the LA directly requesting you would like CENMAC involvement.
Step 1
Review:
> Southwark’s CENMAC Assessment Criteria
> Southwark’s CENMAC Guidance for Schools and Settings
Step 2
Complete and submit the Southwark CENMAC Referral Form with supporting documentation to: cenmac@southwark.gov.uk
Step 3
The Local Authority will review the referral against Southwark’s CENMAC assessment criteria.
Step 4
Where the referral meets the criteria, it will be progressed to CENMAC and additional information will be requested to support assessment planning.
